Important information for your first visit:

1) Please arrive 15 minutes before your first scheduled appointment to register at the front desk.

2) Bring your doctor’s prescription and insurance card.

3) Expect to be at our clinic for approximately 45 - 60 minutes.

4) Bring or wear clothing such as a tank top, shorts and tennis shoes.
